will be given, based upon availability. New patients are always welcomed.
Emergencies
You can call both offices at any time for your emergencies. Should the office be closed, the answering machine will give you a number to contact.
Cancellations If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient. Patients will be billed for late cancellations or no-shows. Please schedule only definite appointments.
Our office accepts with most dental insurance plans. Patients are responsible for deductibles, co-payments and remaining balances. Patients also have the option of paying the dental fees in advance and then receiving their insurance reimbursement check directly from the insurance company.
Financial Policy Co-payments and co-insurances are due at the time services are rendered. Forms of payment accepted by the office are check, cash, insurance assignment and any major credit cards including Diner's Club
The office also accepts the option of payment through SignatureWeb Medical-- a financing program for elective medical procedures. Patients may be pre-approved for SignatureWeb Medical Financing with a low-interest monthly payment plan including 24 or 36 monthly payments with no penalty for early payment. For extensive treatments, payment plans may be arranged with the office manager. Patients may review a treatment authorization form and financial agreement before treatment is initiated. For crowns, bridges and other major work, full payment must be made prior to completion of the treatment.
If you have medical questions which only the dentist can answer, a message will be taken and your medical file will be given to one of our dentists who will return your call the same day. If your question is non-medical, a qualified staff member will be pleased to assist you. |
